Eligibility Requirements
To qualify for our transportation services, you must:

Be able to get in and out of the vehicle on your own

Be able to walk independently or with the aid of a walker

Not be on Medical Assistance

Utilizing transportation for a medical appointment or essential need in Duluth or the surrounding area

Require transportation to a medical or essential appointment in the local area.
Transportation
Scheduling Your Ride
Individuals must be enrolled as a client prior to requesting and receiving transportation services. To ensure that transportation is available on the date and time you need, please follow these steps:
1. Call Age Well Arrowhead
Call Age Well Arrowhead at least 5-7 business days before your scheduled appointment to arrange transportation.
2. Share Specific Appointment Details
Inform our Volunteer Coordinator about your transportation needs like date, time, and pick-up/drop-off location.
3. Be Ready On The Day Of Your Appointment
On the day of your appointment, your driver will pick you up, take you to your appointment, and wait to bring you back home.
